HR Ahmad Aga Khan University started in 1983 with four units: School of Nursing – Hospital – Medical College uniquely winged with Basic Science PhD faculty and Community Health Sciences under the Aga Khan Hospital and Medical College Foundation. It received the charter of a University by then the president of Pakistan in 1985. This then set the pacemaker of turning a medical college into medical universities in the country. Prof. HR Ahmad Historically, the country is sailing the Lord MacClauay model of 1859: English Medium for upper class, Urdu medium metric system for middle class and anti modernity Arabi medium Madressa for Lower Class. This colonial model is being practiced as of today. This in turn drives the three layered classed system that is sailing along the silvery moon. All universities in seventy nine of existence years have not produced more than 5% of doctoral degrees. Thus, on the list are Bachelor-, Master-, and MPhil-degrees in mainly natural sciences and to a lesser degree in social sciences. We experienced the first watershed of history of Pakistan in 1971. Then in 1979, the country was transformed into the frontline state. It was being continued to be doled further for Afghan War and War on Terror. Indeed, the gunpowder mercenaries status blocks the generation of new knowledge. Furthermore, the replacement of the political wheel by religion, which disables a country to ever form the people-based government. How to wake up the spirit of public service is a challenge. This is only possible when political wheel should be the public, while the religion should be a private option. Such a secular state with the public souvernity can finally flatten the medieval classed system to enter into the league of public serviced nations. Life is a journey and every human being is a new beginning to unfold one's creative energy! The Community Health Sciences units should be sought to raise the standard of public health service. If we walk over the world map and learn how many countries are focusing on PUBLIC HEALTH CARE PHC vs PRIVATE DISEASE CARE PDC would be an interesting study. In Pakistan, the focus is on the individual disease care because it generates money. Thus, one finds more hospitals than public health units. In Europe inter alia, it is reversed being public health service oriented. Zebra fish can repair the retina upon injury, but we not! Elephants don't suffer from cancer but we yes? Two millions years ago, we were many species but today left with only Homo sapiens after having flirted with Neanderthal and Denisovans. How this phenomenon could be explained? We being visual being, have vulnerable tender eyes with cornea, lens, renewable aqueous humour, a life time fetal vitrous bubble, retina and optic nerves networking with billions of neurones in brain! The images on the optic nerves connect us with the universe around to be enlightened. The first change from the healthy status is delta zero. The second delta is compensated without symptoms. The third delta is decompensated to noticing symptoms. The fourth delta is in crisis seeking ER. The fifth delta is causing the multi organ failure to be in ICU. It is now being zero kilo-meter away from the Deity of an individual choice. The current practice in West is quality return home or quality departure from the world. How novel if we could serve the humanity earlier during the progression before the manifestation of a disease being a PHC physician. The trio of intellectual companionship, culture exchange and social responsibility is meant to drive the wheel of energy, economical, political, and social change, development and progress enabling pluralism and inclusion.
